<p> Surgical castration by orchidectomy   <BR><BR>Surgical castration is the simplest and cheapest way to treat metastatic prostate cancer. The obvious disadvantage is the psychological effect of the loss of the testicles.   <BR><BR>LHRH-analogues  <BR><BR>LHRH-analogues and oestrogen achieve a &#8220;medical castration&#8221; by stopping the testicular production of testosterone. LHRH-analogues are injections that have to be given monthly or three monthly for the rest of the patient&#8217;s life. They are effective but very expensive.   <BR><BR>Oestrogen  <BR><BR>Oestrogen can be taken orally on a daily basis. It has a high incidence of thrombotic complications such as stroke and myocardial infarction.  <BR><BR>Anti-androgens  <BR><BR>Anti-androgens oppose the action of testosterone by blocking the androgen receptors. The incidence of erectile dysfunction is less than with surgical or medical orchidectomy because testosterone levels are maintained in the bloodstream. Anti-androgens alone are probably not adequate treatment for metastatic disease. Total androgen blockade by a combination of steroidal anti-androgens and LHRH-analogues or orchidectomy has not been shown to be better than LHRH-analogues or orchidectomy alone. However, non-steroidal anti-androgens yields slightly better results than castration alone.  <BR><BR>Locally advanced disease without metastases.   <BR><BR>The overall results of treatment of patients with disease beyond the prostate are not good. Some patients with early disease beyond the prostatic capsule, and no evidence of metastases, benefit from radical treatment. The most widely used treatment regimens consist of a combination of radiotherapy and hormonal treatment. Treatment options for locally advanced and metastatic disease.   <BR><BR>  Early hormonal treatment   Watchful waiting with hormonal treatment once symptoms develop Disease that has spread to the seminal vesicles and beyond is not real curable.  <BR><BR>Prostate cancer is dependent on the male hormone testosterone. 80% of patients will respond to hormonal treatment that deprives the tumor of testosterone. This response usually involves the shrinkage of metastases and symptomatic improvement for the patient. The response to hormonal treatment is not a cure but can last for many years in some patients. The average duration of response is 2 years. Most cancers eventually escape hormonal manipulation. This is referred to as hormone independent disease and is usually followed by death within a few months.  <BR><BR>Controversy exists regarding the timing of hormonal treatment. Most studies indicate a survival benefit for early rather than late hormonal maneuver. Testosterone deprivation has side effects like erectile dysfunction, breast enlargement and osteoporosis. The earlier hormonal treatment is instituted the greater the chance of complications. <p> 1 Testosterone and its active metabolite. Dihydro-testosterone are essential for prostate cancer to develop, but does not actually cause prostate cancer. Men who are castrated at a young age do not develop prostate cancer.  <BR><BR>2 Prevalence of prostate cancer  <BR><BR>One in ten men will develop clinically significant prostate cancer in their lifetime. It is the most commonly diagnosed cancer in American males with 250 000 new cases reported annually. Prostate cancer is second only to lung cancer as a cause of cancer death in both the USA and the UK. Prostate cancer is rare among Orientals. It is more common in black than white Americans. The disease appears to present at a younger age and behave more aggressively in American blacks. Prostate cancer is common in South Africa and probably underreported as a cause of death. The exact incidence in South Africa is not known as no large-scale epidemiological studies have been performed. It is uncertain whether prostate cancer is more common in South African blacks as compared to whites. In very old men prostate cancer is not always clinically significant. Autopsy data indicate a 70% incidence of prostate cancer in 80 year old men. The majority of these men died with rather than from prostate cancer.  <BR><BR>3 Causes of prostate cancer  <BR><BR>There is no single cause of prostate cancer. The cancer originates in the epithelial cells of the glandular elements of the prostate. As with most cancers defects in the DNA of the cell are central to the development of prostate cancer. Multiple DNA defects are required for cancer to develop. This multi-step process takes place over time. Some defects can be inherited, while others are acquired during the patient&#8217;s lifetime. Prostate cancer is exceedingly rare before the age of 40, but 1 in 8 men between the ages of 60 and 80 years suffer from the disease. 9% of all prostate cancers are caused by a genetic susceptibility, probably inherited via chromosome 1. These genetically related cancers tend to present at a relatively younger age.  <BR><BR>4 What is prostatitis?  <BR><BR>Prostatitis means &#8220;inflammation of the prostate&#8221;, and is one of the most common reasons men visiting the doctor in the western world. It is most common in men over the age of 30, and particularly in men over the age of 60. While prostatitis is treatable, diagnosis can be lengthy and not all treatments are successful. This is partly because the various causes of prostatitis are not fully understood. There are three main types of prostatitis:  <BR><BR>* Acute prostatitis, which develops suddenly and may not be permanent. * Non-bacterial prostatitis, which may develop suddenly or follow a slower or variable course. It is now more commonly called chronic male pelvic pain syndrome because it cannot be proved to be &#8220;non-bacterial&#8221;, though a bacterial cause cannot be identified. * Chronic (bacterial) prostatitis, which develops gradually and may only have subtle symptoms, though it often continues for a prolonged period. *  asymptomatic inflammatory prostatitis - which has no symptoms at all but results in an inflamed prostate, is sometimes mentioned. It has been discovered when biopsies are conducted on the prostate to rule out cancer, and no cancer is found. This is a histological and not a clinical diagnosis. Prostatitis is often reported on the histological analysis of TURP specimens when the prostate resection was performed for symptoms of BPH. <p> Hydrogenation is a chemical reaction &#8212; widely used in the processing of cooking oils and fats &#8212; that turns unsaturated fatty acids into saturated ones. Technically speaking, during this process unsaturated bonds between carbon atoms are reduced by attaching a hydrogen atom to each carbon. Simply speaking, hydrogenation is a process in which hydrogen atoms are added to vegetable oils.  </p>
<p>When the process is not carried out completely (which is common in industry) the ending product is described as being &#8220;partially hydrogenated.&#8221;  </p>
<p>According to Udo Erasmus, author of &#8220;Fats That Heal, Fats That Kill&#8221; &#8220;So many different compounds can be made during partial hydrogenation that they stagger the imagination. Scientists have barely scratched the surface of studying changes induced in fats and oils by partial hydrogenation.&#8221;  </p>
<p><b>Why are Foods Hydrogenated?</b>  </p>
<p>The French chemist Paul Sabatier discovered the hydrogenation process back in 1897. However, it wasn&#8217;t until W. Normann, an Englishman, received a patent in 1903 for the hydrogenation of liquid oils using hydrogen gas that the process became part of industry worldwide.  </p>
<p>When a food is hydrogenated, its molecular shape changes, making it more solid and rigid. An oil, for instance, that is hydrogenated will become solid, even at room temperature (such as hard margarine or shortening).  </p>
<p>There are two major reasons why foods are hydrogenated, and they both boil down to dollars and cents:  </p>
<p>It increases the shelf life of foods.  </p>
<p>It increases flavor stability in foods.  </p>
<p>Food manufacturers, therefore, love to use hydrogenated oils and fats because their foods stay fresh and good-tasting much longer than any natural food ever could.  </p>
<p><b>The Big Problem</b>  </p>
<p>&#8220;Hydrogenation, which is used to turn oils into margarine, shortening, or partially hydrogenated vegetable oil, produces trans-fatty acids, which are twisted molecules. Twisted, their shape changes, and they lose their health benefits and acquire toxicity instead,&#8221; says Erasmus.  </p>
<p>The creation of trans fats is the major, and until recently overlooked, health risk of eating hydrogenated foods. In fact, major may be an understatement.  </p>
<p>So toxic are these fats that the National Academy of Sciences&#8217; Institute of Medicine said that trans fats shouldn&#8217;t be consumed at all. The Food and Drug Administration says that intake should be as low as possible.  </p>
<p>Yet, unknowingly, some Americans eat 30 to 40 grams of trans fat daily.  </p>
<p>According to the Harvard School of Public Health, trans fats:  </p>
<p>Double the risk of heart attack  </p>
<p>Are responsible for the deaths of 30,000 Americans every year  </p>
<p>Increase the risk of diabetes  </p>
<p>Other research has shown that trans fats:  </p>
<p>Raise your body&#8217;s level of bad cholesterol (LDL) while lowering the good cholesterol (HDL)  </p>
<p>Increase triglycerides and inflammation  </p>
<p>Interfere with vision in children  </p>
<p>Hinder liver detoxification  </p>
<p>Correlate with increased prostate and breast cancers  </p>
<p>Impede insulin function  </p>
<p>Interfere with reproduction in animals  </p>
<p>Trans fats are found in a wide range of processed foods from the obvious (fried foods, margarine, baked goods, vegetable shortening) to the unexpected (bread, cookies, snack crackers, salad dressings, granola bars, cereals, frozen dinners and much, much more).  </p>
<p>In fact, trans fats can be found in 40 percent of all processed foods in supermarkets today.  </p>
<p><b>The Good News</b>  </p>
<p>As of January 1, 2006, new FDA regulations require food manufacturers to list trans fats on food nutrition labels (directly under the line for saturated fat). So, as a consumer, you can now avoid trans fats much more easily, just by reading nutrition labels. If you also read ingredient lists, terms to watch out for include anything that says hydrogenated or partially hydrogenated, along with vegetable shortening or margarine, which may also be hydrogenated. </p>

<p> The nurse was persistent. She reminded me of a Scottish terrier, which once it finds what it&#8217;s looking for, growls at it and refuses to let go. &#8220;Trust me, it&#8217;s in your own interests. Every man should come to see us at least once a year.&#8221;  <BR><BR>&#8220;Okay, I&#8217;ll think about it.&#8221;  <BR><BR>That was like a red rag to a bull. &#8220;No, don&#8217;t THINK about it. Just do it.&#8221;   <BR><BR>Then she hit me with a guilt trip. &#8220;You should think yourself lucky. Many men would be glad of this opportunity! I&#8217;ll put you down for Thursday morning at 9.15.&#8221;  <BR><BR>There was no arguing with that . . .  <BR><BR>So what was all the fuss about? Well, I hadn&#8217;t attended my doctor&#8217;s surgery for over a year, so they thought it was time I was reined in. They run a &#8216;Well Man&#8217;s Clinic&#8217; these days and they&#8217;re very proud of it.  <BR><BR>When I got there the nurse was still shaking her head and &#8216;tut-tutting&#8217; like a mother hen. Again she hit me with her irrefutable logic. &#8220;You put your car in for a check every year, don&#8217;t you? So what&#8217;s more important, your body or your car?&#8221;  <BR><BR>And believe it or not, the &#8216;overhaul&#8217; was really quite simple.  <BR><BR>She asked a few questions, ticked a few boxes, did some routine tests like checking my blood pressure, then - ouch! - took a sample of blood from a vein in my left arm.  <BR><BR>Not a word of sympathy! I got the distinct impression she&#8217;d had all she could take from men who never think about their health, but who create a fuss when they walk through the clinic door.  <BR><BR>&#8220;This will be divided into smaller samples,&#8217;&#8221; she said matter-of-factly, &#8220;then sent off for analysis.&#8221;  <BR><BR>The next part really impressed me. The nurse reeled off a whole list of things that would be tested from that one blood sample: thyroid activity, glucose levels, PSA level (which measures prostate activity), cholesterol level, blood count . . . and so on.  <BR><BR>So far, so good. A few days later I was summoned back to the surgery for the results. I must admit, that day I was more than a little worried and had to force myself to keep the appointment. But I came away walking on air!  <BR><BR>The results were normal. My blood pressure was as it should be, there was no sign of diabetes or high cholesterol. All fears had been groundless. I felt great.  <BR><BR>Yet when I reached home, my heart was heavy.  <BR><BR>You see, my own Dad died over ten years ago. His was a slow and painful death from prostate cancer. It deeply affected us all, but more especially his grandchildren, who adored him.  <BR><BR>I&#8217;ll never forget the sad look on the face of the consultant as he told my mother: &#8220;If only we&#8217;d caught this sooner&#8230; It needn&#8217;t have come to this, you know.&#8221;  <BR><BR>If only, if only &#8230;  <BR><BR>If only there had been a &#8216;Well Man&#8217;s Clinic&#8217; in my Dad&#8217;s day. If only he&#8217;d had the sense to have a medical check up every so often.  <BR><BR>But who am I to criticise? My Dad was a typical man of his generation: - ignore health problems and they&#8217;ll go away!  <BR><BR>So what was my excuse? Despite my Dad&#8217;s experience, despite the fact that I want to be here for my own family, for the children I teach and the ones I write my books for, I had to be practically dragged, kicking and screaming, for a routine check-up. Apparently I&#8217;m not alone. It would appear there are millions of men around the globe who HAVE clinics and check- ups readily available, but who won&#8217;t go for a simple &#8216;physical&#8217;. They&#8217;ll wait until problems are far advanced - and in many cases too far advanced!  <BR><BR>Believe me, it feels great to know that your health is fine and that if any subsequent tests reveal a problem, it will be in the early stages and can be treated easily and effectively.  <BR><BR>I now feel like shouting the good news from the roof tops - but I&#8217;ll have to make do with the internet:   <BR><BR>Fellow Dads, get your heads out of the sand! Bite the bullet! Gather up your courage! Use whatever metaphor or image you like, but take yourselves down for &#8216;a physical&#8217; and look after your health.  <BR><BR>Follow the example of our wives and partners, who show sound common sense when it comes to health matters.  <BR><BR>You owe it to yourself.   <BR><BR>But more importantly, you owe it to your loved ones &#8230;  <BR><BR>Happy Parenting!   <BR><BR>About the author:<BR><BR> Worried about your family? <p>Once only associated with strong bones and teeth, new scientific research is finding that Vitamin D can cut your risk of developing common cancers by as much as 50%. </p>
<p>Says Sara Hiom, head of health information at Cancer Research UK,  There is evidence to suggest that the vitamin (Vitamin D) plays a role in keeping cells healthy.&#8221; </p>
<p>Researchers in the U.S. have found that the  natural  form of Vitamin D, D3 or cholecalciferol, can dramatically cut our chances of getting breast, ovarian, colon, and prostate cancers by up to 50%. </p>
<p>As a matter of fact, researchers are telling us that they have known about the link between Vitamin D and a decreased cancer risk for years. </p>
<p>Well my question to them is this  what has been taking you so long to tell us? </p>
<p>They say that up until just recently, they did not know the levels of Vitamin D necessary to provide this decreased risk. </p>
<p>What current research is now finding is that the optimum amount of Vitamin D necessary to reduce your risk of developing these cancers is 1000 IU. This is two and a half times the Recommended Daily Allowance of 400 IU set forth by the Food and Nutrition Board of the Institute of Medicine here in the U.S. </p>
<p>Many experts now believe that the current RDA of 400 IU for an adult is too low and does nothing more than help ward off chronic bone diseases such as rickets and osteoporosis. </p>
<p>Recently, an article appearing in the American Journal of Public Health, after reviewing 63 independent studies, concluded that thousands of lives might be saved each year and thousands more cancers prevented by folks taking supplemental Vitamin D. </p>
<p>Professor Cedric Garland of the University of California in San Diego who led a major study on the relationship between Vitamin D and cancer risk said this,  A preponderance of evidence, from the best observational studies the medical world has to offer has led to the conclusion that public health action is needed.&#8221; </p>
<p>This public health action he advocates is the education of the public concerning Vitamin D. </p>
<p>What form of Vitamin D is Best? </p>
<p>Remember that your body has the ability to manufacture its own Vitamin D through direct exposure to sunlight. This form of Vitamin D is called Vitamin D3 or cholecalciferol. This of course is the best form of the vitamin. </p>
<p>The synthetic form is Vitamin D2 or ergocalciferol. This is what is used to fortify milk for instance. It is estimated that it takes twice as much D2 to get the same benefit as Vitamin D3. </p>
<p>Vitamin D3 can also be found in oily fish such as salmon (3 ounces = 530 IU), sardines (3 ounces = 231 IU), and Mackerel (3 ounces = 214 IU). </p>
<p>Unfortunately not all of us can get the required amount of sunlight necessary to produce enough Vitamin D3. </p>
<p>Some of us have fair skin and need to use sunscreen when we venture outdoors. Some of us have dark skin and do not produce as much Vitamin D3 due to our skin pigment. </p>
<p>And of course, the cold parts of the year can keep most of us indoors or bundled up when we do head outside. </p>
<p>And also unless you eat lots of oily fish, you are not going to get your 1000 IU there either. </p>
<p>Professor Garland warned about the dangers of overexposure to the sun,  Dark-skinned people, however, may need more exposure to produce adequate amounts of vitamin D, and some fair-skinned people shouldn&#8217;t try to get any vitamin D from the sun. The easiest and most reliable way of getting the appropriate amount (of Vitamin D) is from food and a daily supplement. </p>
